Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primary responsibility of the IDDT Case Manager will be the provision of case management services to adults with recurrent mental health and substance abuse disorders. Specific duties include but not limited to:

  • conducting goal-directed community-based activities for adults with recurrent mental health and substance abuse disorders to assist the client in securing access to and mastering the use of community services and resources that will both reduce or eliminate the risk of hospitalization and support independent living in the community;
  • coordinating the services listed above with other case managers, IOP staff, acute care staff, Access Center staff, medication services staff, homeless case management staff, and other community providers by communicating with such staff and community providers on a regular basis and coordinating schedules and assignments;
  • providing outreach and crisis intervention services to clients and prospective clients to enable timely access to those services appropriate and necessary for crisis stabilization with the purpose of diverting the client from hospitalization and allowing the client to maintain independent living;
  • completing all paperwork associated with the financial, clinical, legal, or regulatory management of the case or BNC's operations in a timely and accurate manner;
  • participating in public information and public relations activities, serve as a community liaison and advocate for mental health and the Bert Nash Center;
  • providing assistance with the evolution of an effective, efficient preventative program of services;
  • regular and predictable attendance is an essential function of the job; and
  • additional responsibilities that may be assigned as circumstances warrant.

Qualifications include but not limited to:

  • a BA/BS degree or four years of equivalent education and/or experience working in the human services field.
  • reliable transportation; and a valid, unrestricted driver's license
  • an ability to work independently and in cooperation with others;
  • ability to exercise sound independent judgment;
  • strong organizational skills;
  • strong oral and written communication skills;
  • strong interpersonal skills in working with diverse publics;
  • flexibility in assigned working hours;
  • an energetic and positive approach to the rapidly evolving changes and challenges of a complex work place.

Salary : $20 - $28
