Admissions Care Navigator

Pay is commensurate with experience beginning at $19.00 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primary responsibility of the Admissions Care Navigator is to meet with clients who present for intake and support them with the completion of a brief mental health screen, registration paperwork and scheduling the intake with a therapist. Admissions Care Navigators will work with active Bert Nash clients who are referred to the team by other Bert Nash treatment providers to address short term care coordination needs. The Admissions Care Navigator will report to the program manager of the Admissions Department. Specific duties include but not limited to:

• Creating new charts in the electronic medical record prior to the intake.
• Support new and returning clients to Bert Nash with screening and triaging of their risk factors to determine the next appropriate steps based on the acuity of their care needs.
• Assist clients with completing registration paperwork and orientation to services, and then documenting the interaction in the electronic medical record.
• Assist clients with paperwork related to receiving services and assist Medical Records with scanning appropriate forms into the chart.
• Review fee policies and billing procedures with clients.
• Supporting clients in crisis with safety planning. If a client cannot be safety planned, walking with clients to the Treatment and Recovery Center and providing a warm handoff to the staff.
• Respond to phone calls and voicemails to the Care Navigator phoneline.
• Following up on referrals to the Care Navigation team. This includes assisting the client with identifying and referring to other community resources and services that they are eligible for, following up with outreach on high risk/high needs clients to obtain more information and possible internal referrals to additional supports and communicating with treatment teams on any changes (i.e., recent hospitalization, or referrals to IOP).

Qualifications include but not limited to:

• BA from an accredited four-year college required and/or related experience in a human service-related field
• A commitment to the principles of partnering with family and community resources
• Commitment to developing cultural diversity in the workplace and in service delivery.
• Strong evaluation, assessment, and customer management skills.
• Knowledge of and skill to provide intervention techniques utilizing proven treatment models and modalities.
• Prior computer experience and basic computer fluency.
• Ability to work independently and in cooperation with others.
• Flexibility in assigned working hours, and willingness to work evenings (until 6pm) as needed.
• Additional responsibilities may be assigned as circumstances warrant.
