Case Manager - Child and Family Services

Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primary responsibility of the Child & Family Services Case Manager will be case management services, resource acquisition, crisis intervention, and outreach activities to specified youth and their families. Specific duties include but not limited to:

• providing case management services, resource acquisition, crisis intervention, outreach activities and coordinate these services with other community health and social service agencies;
• delivering services in a variety of locations, including but not limited to the client's home, the school, the Child & Family Services program site, or another community setting, on an as-needed basis;
• providing transportation to and from various activities or appointments as indicated by the treatment plan or as instructed by the QMHP;
• providing services on an as-needed basis both with respect to time-of-day and location;
• delivering reimbursable hours of service compliant with established standards;
• responding to crises while on call in rotation with other similarly trained staff.

• regular and predictable attendance is an essential function of the job.

Qualifications: The Child & Family Services Case Manager position requires but not limited to:

• a BA/BS degree or four years of equivalent education and/or experience working in the human services field.
• reliable transportation and automobile insurance; and a valid, unrestricted driver's license;
• prior computer experience and basic computer fluency;
• must be able to achieve within one's first 6 months of employment a working competency in the management and use of the Center's electronic medical record (EMR) software as demonstrated by timely and accurate documentation of the clinical record;
• demonstrated strong interpersonal skills in working with diverse publics, consumers, and referral agents;
• flexibility in assigned working hours;
• a commitment to interdisciplinary and community mental health center approaches; and
• an energetic and positive approach to the rapidly evolving changes and challenges of a complex workplace.
