The Impact You'll Make (Your Core Responsibilities)
As a Berkley Hospice RN Case Manager, you are the champion of the patient experience. You will manage a meaningful caseload (home and facility-based) by:
- Clinical Leadership: Performing expert care coordination, advanced symptom management, ongoing assessment, and complex clinical care.
- Patient Advocacy: Serving as the key contact to ensure patient comfort, dignity, and a high quality of life.
- Relationship Building: Establishing strong, trusting bonds with patients, families, and the Interdisciplinary Group (IDG) team.
- Collaborative Care: Partnering closely with our dedicated social workers, chaplains, certified aides, and clinical leadership to deliver holistic support.
What We Need From You (Qualifications)
We are looking for a Heart-Centered RN ready to invest in our mission:
- Active, unencumbered Colorado RN license.
- Minimum of 1-2 years of clinical experience; Hospice, Palliative Care, or Home Health experience is highly preferred.
- Dependable, adaptable, and a skilled communicator who thrives in a team-based setting.
- Valid driver's license and reliable transportation for travel within the South Metro service area.
