Job Summary:
We are seeking a highly motivated and organized PAS Care Manager to join our team. In this role, you will play a crucial role in coordinating patient care, collaborating with healthcare professionals, and ensuring optimal patient outcomes. This position requires strong communication, organizational, and problem-solving skills, as well as a passion for patient-centered care.
Responsibilities:
- Patient Care Coordination:
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement comprehensive patient care plans.
- Monitor patient progress, identify potential problems, and adjust care plans as needed.
- Provide education and support to patients and their families regarding their health conditions and care plans.
- Communication and Collaboration:
- Maintain clear and effective communication with patients, families, and healthcare team members.
- Facilitate communication and collaboration among different departments and disciplines.
- Participate in team meetings and rounds to ensure coordinated care
- Documentation and Record Keeping:
- Maintain accurate and up-to-date patient records and documentation.
- Ensure compliance with relevant regulations and standards.
- Resource Management:
- Identify and access appropriate resources for patients and their families.
- Advocate for patient needs and ensure access to necessary services.
- Other Duties as Assigned:
- Perform other duties as assigned by the supervisor.
Qualifications:
At least two years of experience in care management, healthcare, or home care settings is preferred.
