Belle Ame PCS are seeking a Nurse Clinician to assist in our company’s mission to provide patient-centered home care to patients with complex conditions. The Nurse Clinician serves as a key member of the healthcare team, oversee and assist in the development of comprehensive assessments and care plans, mentoring staff, and implementing evidence-based practices to improve patient outcomes.
Key Responsibilities
- Direct Patient Care: Perform advanced assessments, diagnostic, and therapeutic procedures for patients with complex, acute, or chronic conditions.
- Care Coordination: Act as a liaison between physicians, nursing staff, and families to manage treatment plans and transitions of care.
- Clinical Leadership & Education: Mentor nursing staff, provide expert clinical guidance, and introduce evidence-based knowledge to improve nursing practices.
- Quality Improvement: Evaluate current clinical practices, ensure compliance with regulatory standards, and initiate safety measures.
Required Skills and Qualifications
- Licensure: Registered Nurse (RN) license.
- Experience: 3-5 years of nursing experience.
- Home Healthcare experience is highly desired.
- Education: Advanced knowledge, often involving specialized training or certification in a specific nursing field.
- Soft Skills: Strong critical thinking, communication, and leadership skills for managing complex care and mentoring teams.
