Make an Impact as a Care Coordinator – Empowering Families, Supporting Youth! Are you passionate about helping youth and families navigate challenges and achieve their goals? As a Care Coordinator at our Comprehensive Services program, you'll play a critical role in supporting and guiding families referred by the Department of Children and Families (DCF). You’ll collaborate with families, youth, and a dedicated team of professionals to create tailored treatment plans, foster meaningful participation, and connect families to essential services that make a real difference. In this role, you’ll not only provide direct support but also be the driving force behind the coordination of care. Your expertise in assessment, treatment planning, and fostering team collaboration will ensure that youth and families receive the services and resources they need to thrive. You'll facilitate team meetings, develop transition plans, and create safety strategies that help prevent crises. Key Responsibilities: Youth & Family Support: Provide comprehensive care coordination for youth and families referred by the Department of Children and Families (DCF), ensuring timely access to services and support. Treatment Plan Development: Conduct assessments and create youth- and family-centered treatment plans, focusing on achievable goals and strategies to address identified needs. Team Coordination: Lead and facilitate team meetings to develop, update, and review treatment plans, ensuring that all team members are aligned with the youth and family's goals. Family Engagement & Empowerment: Foster active participation from youth and families in the treatment planning process, ensuring they are informed, engaged, and empowered to make decisions about their care. Resource Navigation: Assist families in accessing available community resources, including mental health services, education, and social supports, to meet the needs outlined in the treatment plan. Safety & Risk Management: Develop and document Risk Management/Safety Plans to prevent crises, ensuring stability and reducing the need for out-of-home services. Coordination with Partners: Collaborate with state agencies, the school system, and other service providers to ensure a coordinated, holistic approach to care. Ongoing Communication & Documentation: Provide weekly updates to DCF and relevant partners, maintaining clear, up-to-date documentation to ensure compliance with program requirements and the delivery of high-quality care. Transition Planning: Work closely with families to develop purposeful, agreed-upon transition plans, preparing youth and families for successful changes in their care. Skills & Abilities: Clinical Expertise: Strong foundation in care coordination, treatment planning, crisis management, and behavioral health interventions. Communication: Excellent verbal and written communication skills, able to effectively engage with families and collaborate with a diverse team of professionals. Time Management: Ability to manage a caseload, prioritize tasks, and address urgent needs while maintaining high standards of service delivery. Cultural Competence: Demonstrates the ability to work with families from diverse cultural, racial, and ethnic backgrounds, incorporating cultural understanding into care and treatment planning. Collaboration & Coordination: Ability to work effectively with multidisciplinary teams, maintaining strong communication and ensuring that all team members are aligned with the youth and family’s treatment goals. Professional Development: Committed to continuous learning and applying new skills, techniques, and feedback in clinical practice. Technology Utilization: Proficient in using agency software, communication systems, and other relevant technologies to support service delivery and coordination of care.
