BHN is currently seeking a Family Partner to join our Intensive Care Coordination (ICC) team to support parents and caregivers of youth with behavioral health needs.

As a Family Partner, you’ll use your lived experience as a parent or caregiver of a child with behavioral health needs to support other families in navigating systems and building resilience.
- Provide peer support to parents and caregivers
- Help families identify strengths and develop strategies to meet care plan goals
- Educate families on navigating child-serving systems such as DCF, DMH, education, and behavioral health
- Empower families by linking them to peer networks and support groups
- Connect families with both formal and community-based resources
- Foster a culturally responsive and inclusive environment
- Maintain accurate and timely documentation
- Collaborate with care teams and attend family planning meetings
- Actively engage in supervision and ongoing training
- Bilingual- fluent in english and spanish
- You have personal experience as a caregiver of a youth with special or behavioral health needs
- You have a High School diploma/GED and at least 2 years of experience working with children/adolescents/transition-age youth
- OR an Associate’s Degree and 1 year of relevant experience
- OR a Bachelor’s Degree and 1 year of relevant experience
- You are knowledgeable about navigating child and family-serving systems
- You have strong communication, organization, and interpersonal skills
- You are comfortable using electronic systems and documentation tools
- You are culturally responsive and value diverse lived experiences
- You have a valid driver’s license and access to a reliable vehicle
