As an Access Navigator, you will serve as a key point of contact for individuals entering treatment, helping guide them through services, assessments, and connections to care.
- Serve as the primary point of contact for individuals, ensuring consistent engagement and follow-through
- Complete intake processes, assessments, and required documentation within the electronic health record
- Collect and utilize Social Determinants of Health (SDOH) data to inform care planning
- Coordinate referrals to internal programs and community-based services
- Collaborate closely with clinical staff, care managers, and external providers
- Maintain consistent communication with individuals throughout their connection to services
- Support navigation into care management and recovery support programs
- Ensure accurate documentation, tracking, and follow-up of all individuals served
- High School Diploma or GED required; Associate’s Degree preferred
- 1–2 years of experience in a healthcare or behavioral health setting required
- Knowledge of substance use, mental health, and care management preferred
- Strong organizational, communication, and customer service skills
- Ability to work with diverse populations in a compassionate and professional manner
- Comfortable using electronic health records and standard computer systems
- Knowledge of community resources and how to access them
- Flexible, team-oriented, and able to manage multiple priorities
