Long-Term Care Discharge Planner

Job Summary: The Long-Term Care Discharge Planner is responsible for coordinating and managing the discharge planning process for patients in long-term care facilities. The role involves collaborating with healthcare professionals, patients, and their families to ensure a safe and effective transition from the facility to home or other appropriate care settings.

Key Responsibilities:

  • Assessment and Planning:
    • Evaluate the needs of patients nearing discharge to determine appropriate post-acute care requirements.
    • Collaborate with the healthcare team to gather information on the patient's medical condition, functional status, and social support system.
  • Coordination and Communication:
    • Communicate with patients, families, and healthcare providers to develop a comprehensive and individualized discharge plan.
    • Coordinate with external service providers, including home health agencies, rehabilitation centers, and community resources.
  • Documentation:
    • Maintain accurate and detailed records of the discharge planning process, including assessments, recommendations, and communication with involved parties.
  • Resource Utilization:
    • Identify and connect patients with appropriate community resources and support services to ensure a successful transition and ongoing care.
  • Insurance and Financial Coordination:
    • Work with insurance companies and financial counselors to address any coverage issues and ensure that patients have access to the necessary services.
  • Follow-Up:
    • Conduct follow-up assessments to monitor the patient's progress after discharge and address any challenges or concerns.
  • Quality Improvement:
    • Participate in quality improvement initiatives to enhance the effectiveness of the discharge planning process and improve patient outcomes.


Qualifications:

Bachelor's degree in Social Work, Nursing, or a related field.

Previous experience in discharge planning or a related healthcare role.

Strong interpersonal and communication skills.

Knowledge of healthcare regulations and reimbursement processes.

Ability to work collaboratively with a multidisciplinary healthcare team.

Salary : $45,000 - $60,000
