Responsibilities:
Discharge Planning:
- Collaborate with the interdisciplinary team to assess residents' needs and create individualized discharge plans.
- Ensure that the discharge process is in compliance with relevant regulations and ethical standards.
- Coordinate with healthcare providers, community agencies, and families to facilitate a smooth transition for residents leaving the facility.
- Communicate discharge plans to residents and their families, addressing concerns and providing necessary information.
- Monitor and evaluate the effectiveness of discharge plans, making adjustments as needed.
- Serve as the primary point of contact for residents and their families regarding grievances, complaints, or concerns.
- Conduct thorough investigations of grievances, working collaboratively with relevant departments to gather information.
- Document and track all grievances, ensuring accurate and complete records are maintained.
- Develop and implement strategies to address and resolve grievances in a timely and satisfactory manner.
- Communicate with residents, families, and staff to keep them informed about the status of grievance resolutions.
- Stay informed about federal, state, and local regulations related to discharge planning and grievance resolution in long-term care facilities.
- Ensure that all documentation related to discharge planning and grievance resolution is accurate, complete, and in compliance with regulatory requirements.
- Collaborate with other departments to provide training on discharge planning and grievance resolution processes.
- Foster effective communication and collaboration with residents, families, healthcare providers, and internal staff.
- Participate in interdisciplinary team meetings to discuss and address resident needs and concerns.
- Work closely with social workers, case managers, and other healthcare professionals to coordinate seamless transitions for residents.
- Previous experience in discharge planning and grievance resolution in a long-term care setting.
- Knowledge of relevant regulations and laws governing long-term care facilities.
- Strong interpersonal and communication skills.

Salary : $50,000 - $60,000
