Job Responsibilities
- Conduct age-appropriate assessments of patients, including history taking and physical examination in accordance with current standards of professional practice, regulatory requirements and agency policies and procedures.
- Perform assessment/data collection in an ongoing systematic manner, focusing on physiological, psychosocial, cultural, spiritual and cognitive status, patient/family education, family involvement, and patient advocacy.
- Consult with other clinicians as dictated by patient need and assessment findings.
- Participate with physicians and other clinicians in the department to assure effective patient care consistently with quality indicators for patients cared for in the designated service.
- Review medical records for follow up or ordering of diagnostic studies under established protocols.
- Provides primary medical care to patients and manages acute and chronic conditions.
- Assesses patients’ conditions and age-specific medical needs; sets goals and prescribes appropriate clinical interventions to meet these goals.
- Prescribes appropriate laboratory and diagnosis procedures related to patients’ health status and facilitates appropriate treatment intervention predicated on results of these procedures.
- Provides comprehensive medical services for members of family, regardless of age or sex, on continuing basis: Examines patients, using medical instruments and equipment.
- Participates in quality assessment and improvement activities and achieves productivity goals. Examination of patients, formulation differential diagnostic plans, defines and orders required diagnostic testing.
- Interpretation of examination findings and test results, and implementation of treatment plans.
- Determines need for consultation and assists in medical care and treatment provided in the direction of other specialists.
- Answering patient telephone consultations with the assistance of clinic staff.
- Orders or executes various tests, analyses, and diagnostic images to provide information on patient’s condition.
- Maintains accurate and detailed medical records about each patient, including observations during examinations (via EMR).
- Administers or prescribes treatments and medications.
- Promote health by advising patients concerning diet, hygiene, and methods for prevention of disease.
- Writes orders to inoculate and vaccinate patients to immunize patients from communicable diseases.
- Refers patients to medical specialists for consultant services when necessary for patient’s well-being.
- Works continually to assure deliverance of quality clinical care in a safe, efficient, competent, and effective manner.
- Prepare records and reports as required in support of services rendered in accordance with established procedures.
- Attend and participate in patient care reports, patient care conferences, team conferences, professional staff conferences and other appropriate professional activities only to the extent that such attendance and participation is relative to his/her assigned cases and/or performance of services as determined by the Medical Director.
- Comply with all laws applicable to and practice within scope of Family Practice, including HIPPA.
- Requires the ability to relate to people of diverse backgrounds, cultures, races, sexual orientations and gender identities or expressions.
- Responsible for maintaining confidentiality of all patients, clients, proprietary, and protected information.
- Employees are accountable for meeting performance standards of their departments. They participate in compliance audits and quality improvement plans.
- Education - provides health teaching to patients and families, participates in development and delivery of educational programs for area health care providers.
- Board Certification
- Current NYS license as Nurse Practitioner
- Completion of a Master’s Program in Adult NP, Family Practice NP
- Current DEA if required for specialty
- Depending on the particular specialty, ACLS and/or NALS and/or PALS or PEARS may also be required for the position
- 1-year related experience with adult population in a clinic setting
- Experience working in community-based health care settings is also preferred
- Experience/An understanding of EMRs is helpful.
- The ability to demonstrate teamwork and effective interpersonal relations skills is imperative.
- Excellent written/oral communication and computer skills are required.
- Ability to foster and maintain a positive working relationship in a multidisciplinary team and clients
