POSITION SUMMARY:

 

The Family Nurse Practitioner to provide quality, competent, safe and compassionate care based on established standards in a patient-focused preventative and treatment care. The nurse practitioner will work closely with a team of professionals to provide culturally sensitive, high quality, comprehensive health services to the community to achieve expected outcomes and comply with BSFHC Mission.

 

ESSENTIAL JOB RESPONSIBLITIES AND DUTIES:

 

Duties and Responsibilities include but not limited to:

 

• Assess, diagnose, and identify the expected outcomes to plan and formulate the care with the provider

and implement patient-centered care and services according to Nursing Standards and Practice.

Analyze and assess patient health by interviewing patients, performing physical examinations,

obtaining, updating, and studying medical histories to determine nursing diagnoses.

• Identify expected outcomes and anticipates unexpected or adverse outcomes as individualized to the

patient and implements interventions identified in the plan of care as needed

• Perform therapeutic procedures by administering injections, immunizations, suturing, managing wounds

and infections.

• Provide continuity of care by developing and implementing patient management plans.

• Protect patients and employees by adhering to infection-control policies and protocols.

• Maintains professional and technical knowledge by attending educational workshops; reviewing

professional publications; establishing personal networks; participating in professional societies.

Develops health care team staff by providing information, educational opportunities, and experiential

growth opportunities.

Contributes to team effort by accomplishing related results as needed.

• Supports the mission, philosophy, standards, goals and objectives of the Center strategic area.

• Participates in coordination of unit activities to provide patient care in a healing, humane and caring

environment.

• Collaborates with the members of the interdisciplinary team and promotes collegial relationships.

• Maintains safe and clean working environment by complying with procedures, rules, and regulations.

• Comply with federal, state, and local legal and professional requirements by studying existing and new

legislation; anticipating future legislation; enforcing adherence to requirements; advising management

on needed actions. Promote self-assessment and participate in designated activities related to

professional standards for professional growth and development and functions as a respectful member

of the health care team.

 

Family Nurse Practitioner JD – BSFHC

 

• Serve as a support and resource for nursing staff, providers and professionals.

• Utilize resources to meet individual learning need and promote professional development in clinical

practice.

• Participate in the unit based quality and performance improvement program consistent with the clinical

service and departmental program.

• Participate in quality improvement programs and activities that promote patient, family, physician and

staff satisfaction. Maintains awareness of current evidence-based practice and uses research

applicable to patient care.

• Maintains documentation of services including service plans, progress notes, medication informed

consent, quarterly treatment plan reviews, and other required information in compliance with agency

and third party compliance standards.

• Maintain excellent documentation of services in EMR including service plans, progress notes,

medication informed consent, quarterly treatment plan reviews, and other required information in

compliance with agency and third party compliance standards

• Ensure documentation of services correlates with appropriate billing codes to optimize billing services

 

 

Patient Care:

• Coordinate care with referral sources and other involved professionals.

• Maintain regular communication with the involved providers in the primary care practice as appropriate

• Considers needs and behaviors of specific patient age and cultural groups in all patient care activities

• Evaluates the patients’ progress toward attainment of expected outcomes.

• Educates the patient and family to facilitate and promote health adjustment to new or changed health

patterns.

• Excellent customer service by responding to other professionals and patients’ concerns, questions, and

calls in a professional and timely manner.

• Coordinate care with referral sources and other involved professionals.

• Educate patients, their families members, and/or significant others relating to crisis intervention,

treatment and progress

• Encourage patients to participate fully in all aspects of their treatment to facilitate recovery.

 

 

EDUCATION/EXPERIENCE REQUIREMENTS:

• Master's Degree in Advance Practice Nursing required

• Board Certification required.

• New York State Registered Nurse and Nurse Practitioner licenses required.

• DEA and BLS required

• 1 year related experience in Adult/Pediatrics

 

Other Related Skills/Experience and Requirements:

• Knowledge of E-Prescribe and EMR systems a must

• Must be flexible with scheduling

• Excellent customer service by responding to other professionals and patients’ concerns, questions, and

calls in a professional and timely manner.

• Must possesses excellent time management skills including; Organization, Multitasking, Prioritization,

and Resourcefulness

• Membership in pertinent professional organizations preferred.

• Experience working in an outpatient setting

• Ability to foster and maintain a positive working relationship in a multidisciplinary team and clients

• Ability to prioritize and to be detail-oriented, multi-task and must strive in a fast-paced environment.

• Excellent verbal and written communication and documentation skills.

• Ability to make critical clinical decisions independently; ability to process information quickly

• Strong medical/clinical understanding of the business of health care.
