Key Responsibilities:
- Manage and complete all Minimum Data Set (MDS) assessments in compliance with federal and state guidelines
- Collaborate with the interdisciplinary team to develop and maintain accurate, resident-focused care plans
- Track and document changes in resident status and coordinate timely updates
- Ensure proper coding under PDPM and optimize reimbursement
- Participate in internal quality assurance initiatives
- Maintain full CMS compliance and support regulatory survey readiness
- No case management duties unless deemed necessary by leadership
Qualifications:
- Active RN license in New Hampshire
- Minimum of 1 year MDS experience preferred
- Strong working knowledge of RAI process and PDPM
- High attention to detail and ability to work both independently and as part of a team
- Familiarity with MDS software and EMRs preferred
- Excellent communication and time management skills
