Role Description
This is a full-time, on-site role based in Philadelphia, PA, for a Housing Medical Case Manager. The primary responsibilities include providing medical case management services, coordinating housing plans for clients, conducting assessments, and ensuring access to necessary healthcare and resources. The individual will work collaboratively with housing counselors, healthcare providers, social services, and community organizations to support clients in achieving stability. Regular documentation, utilization management, and advocacy will also be key aspects of this position.
Qualifications
- Proficiency in Medical Case Management and Case Management practices
- Skills in Disability Management and Case Management Services to address client needs
- Experience with Utilization Management to ensure effective coordination of care
- Ability to effectively communicate and collaborate with clients, providers, and community organizations
- Strong organizational and documentation skills
- Knowledge of HIV/AIDS and related health issues; experience working with underserved populations is a plus
- Bachelor’s degree in Social Work or a related field preferred
- Valid driver’s license and reliable transportation for on-site and field visits
