PURPOSE OF THE POSITION
About the Role: Beatrice Loving Heart is seeking passionate Support Planners to provide essential support and care coordination for individuals in the community. Key Responsibilities
Assess clients' needs, strengths, situations, and support networks to develop person-centered Plans of Service.
Serve as the primary point of contact for clients, conducting telephonic care coordination, information gathering, and ongoing monitoring of service plans.
Manage a caseload of up to 55 Clients.
Promote positive health behaviors and educate clients on self-direction principles, philosophy, and resource access.
Establish telephone and on-site contact with clients, families, and caregivers to coordinate services.
Collaborate with Social Workers, Registered Nurses, and community agencies to address complex medical/psychological needs and facilitate transitions to community living.
Access internal/external resources, resolve crises using critical thinking, and ensure optimal client outcomes.
Manage multiple priorities, document activities per contractual billing guidelines, and adapt to policy changes.
Uphold organizational core values, mission, honesty, and integrity.
Qualifications
Bachelor’s degree in health/human services or related field (e.g., psychology, sociology, counseling, social work, physical therapy).
Strong oral and written communication skills; ability to communicate effectively with individuals of varied cognitive abilities and build professional relationships.
Proficiency in Microsoft Office Suite (Word, Excel, Access, Outlook, PowerPoint) and computer-based research.
Ability to read/interpret documents (e.g., assessments, manuals) and write routine reports/correspondence.
Valid Maryland driver’s license, reliable transportation, and willingness to conduct on-site visits.
Self-motivated with strong ability to work independently, manage schedules, prioritize tasks, and handle multiple responsibilities.
