Coordinator – New Jersey Supports Coordination.

- Bachelor’s Degree or higher and 2 years of experience in developmental disabilities or care management/coordination in any services field
- 1 year supervisory experience preferred.
- State and Federal Criminal Background checks and Central Registry check at time of hire
- Successfully complete trainings required by DDD before rendering any service
- Provides operational oversight and supervision to assigned programs with regular communication and interaction with Director;
- Directly supervises NJSC Supervisors including coaching, counseling and disciplining employees, planning, monitoring and appraising job results.
- Assist Director in development of internal policies and procedures that are related to their assigned program.
- Develop and deliver marketing presentations/materials for program expansion.
- Monitors for quality service delivery
- Addresses family/individual and provider concerns
- Attend outside trainings and webinars.
- Provide training to Supervisors and Supports Coordinators
- Ensures compliance with all regulating agencies and internal policy and procedures.
- Review/allocate cases and adjust caseloads as needed.
- Ensure fiscal integrity of program finances including regulation of staffing and overtime.
- Evaluates program resources and identifies areas of strength and weakness.
- On-call coverage for assigned areas;
- Provide caseload coverage as needed.
- Facilitates and ensures all report deadlines are met;
- Understands internal and external policies and follows employee rules of conduct and performance as set forth by Keystone;
- Promotes community awareness by demonstrating responsible, courteous behavior and a positive perception of individuals with developmental disabilities;
- Implements Keystone’s mission statement.
- Ensure compliance with all Division policies, including provisions related to zero-reject and zero-discharge, confidentiality, etc…
- Ensure that any concerns related to individual health and safety of any program consumers are addressed and communicated to all appropriate parties.
- Complete all modifications and updates to the Service Plan as necessary and submit them to the WAC within defined timeframes
- Remain informed of current services, supports, provider pool and best practices
- Ensures that NJSC’s are adhering to all guidelines in the Provider Agreement and the Supports Program Policy Manual
- Office environment combined with Program environment, which includes locations within the community.
- May experience exposure to body fluids and the need to provide personal, private care to individuals;
- Varied shift hours will be required;
- Driving up to 3 hours or more for individual, family and or provider meetings
