Position Summary
Serves as a member of the Healthcare Home team. Position services as part of the care coordination for Healthcare Home encompasses assisting clients in applying for MoHealthNet and other benefits, scheduling appointments, providing resources to clients, engaging in community events, and tracking client data.

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
Serves as a member of the Healthcare Home team. Position services as part of the care coordination for Healthcare Home encompasses assisting clients in applying for MoHealthNet and other benefits, scheduling appointments, providing resources to clients, engaging in community events, and tracking client data.
Essential Duties And Responsibilities
The essential functions include, but are not limited to the following:
- Contacts referred consumers within policy guidelines to begin provide assistance in applying for MoHealthNet and/or other benefits
- Maintains broad knowledge of Missouri Medicaid coverage/guidelines and the timeline of key implementation dates.
- Maintains broad knowledge of community resources and assists consumers in accessing and applying for resources to meet basic needs such as housing, food, emergency assistance, medical care, transportation, etc.
- Assists with benefits applications and tracks progress of applications completed and approved.
- Provides outreach as needed to engage referred consumers who will benefit from applying for MoHealthNet.
- Educates consumers about the benefits of MoHealthNet and the process for obtaining and maintaining benefits.
- Arranges and coordinates identified services needed to assist consumers in securing other needed services and resources. Provides follow up to ensure consumer is able to connect with appropriate services.
- Acts as the Cyber Access liaison with the Division of Behavioral Health.
- Acts as the secondary liaison to Swope Health Maple Woods services. Assists consumers in accessing primary and specialty care needs.
- Participates with post hospital discharges for individuals enrolled in HCH.
- Facilitates the multi-disciplinary teams’ reviews of monthly care management and hospital admission reports. Documents and notifies appropriate team members of Hospitalization/ED admission.
- Uses health information tools (AvatarNX, CareManager, Cyber Access, KHIN, CIMOR and other designated systems).
- Assist with DM3700 outreach.
- Provides outreach on appropriate HCH, Swope PCP, and Tobacco Cessation referrals.
- Assists with Nurse Care Manager appointment scheduling and tracking individuals’ services.
- Provides phone outreach with identified clients who have not been engaging in services.
- Runs weekly status reports to track client data and identify client trends.
- Provides consultation to other agency departments on benefits, community resources, and updates regarding latest resource information.
- Maintains productivity expectations per agency standards.
- Completes progress notes in AVATAR within 72 hours of services provided.
- Coordinates with other organization teams to promote consumer wellness and connection to resources.
- Maintains knowledge of agency technology programs and/or systems needed to do the job and use them effectively.
- Tracks and assimilates data used in coordination of care.
- Supports marketing and public awareness initiatives with community involvement and promotes Beacon programs and services.
- Coordinates outreach events, presentations, and community education programs.
- Maintains required training and certificates.
- Attends and participates in bi-weekly team meetings.
- Maintains a valid driver’s license and reliable transportation.
- Other duties and/or responsibilities may be assigned by supervisor.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
- A combination of 4 years of education and experience is required.
- High School Diploma required
- Bachelor’s Degree in related field, preferred
- Active driver’s license and minimum state-required auto-insurance
- CPR/First Aid certification
- 1 to 3 years minimum work-related experience required
- 3 years minimum work-related experience preferred
- Knowledge of behavioral health diagnosis and symptoms
- General knowledge of MoHealthNet, social security and other benefits, community resources and/or the willingness to research as needed
- Intermediate to Advanced knowledge of Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, and Publisher)
- Excellent communication skills, both written and verbal
- The ability to use and troubleshoot basic business equipment
- The ability to interact with a wide range of individuals
- Strong ability to multi-task with a high level of accuracy and timeliness in project output
