Essential Functions:
- Outreach and recruitment of participants for the HEART program.
- Establish and maintain a trusting and professional working relationship with families by providing regular contact.
- Record all group and individualized services and activities in a complete, concise, and timely manner utilizing program forms into Salesforce (BCFS) databases.
- Maintain a list of applications' status and updated the Salesforce database.
- Provide safe, secure, and supervised transportation for the service population assigned to this position for transport.
- Provide referrals to participants based on needs identified while conducting outreach efforts.
- Assist with coordinating and promoting HEART sponsored events and meetings, including classes and community events.
- Build collaborative relationships with the community, health care providers, community leaders, service agencies, policymakers, funding sources, and target audiences.
- Facilitate meetings and/or training for program participants.
- Communicate consistently and effectively with the program director regarding any concerns or issues relating to community outreach efforts.
- Maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook, including demonstrating respect for agency staff, families, and community members and
Measurable Deliverables:
- Documentation entered accurately within two business days.
- Conduct community outreach a minimum of three days a week.
- Attend community events at lease 2 times per month.
- Demonstrate the ability to:
- Respond sensitively and competently to the service population’s cultural and socio-economic characteristics.
- Communicate effectively in writing and verbally in English.
- Work in a fast-paced environment.
- Maintain computer literacy required to meet the responsibilities of the position.
- Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
- Travel as needed.
- Other job duties as assigned per grant requirements.
- Maintain certificates required for the position including but not limited to First Aid and CPR.
- Valid Texas Driver’s license with a clear driving record and reliable transportation.
- Pass a pre-employment drug screen and random drug screens.
- Provide proof or work eligibility status upon request.
- Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).
