POSITION SUMMARY:
The Community Coordinator will provide specialized administrative and technical support to build and maintain community relationships with partnering organizations.
ESSENTIAL RESPONSIBILITES:
- In coordination with the Regional Operations & Project Manager, the Community Coordinator will work to develop a consortium of organizations working together to better meet the needs of the patient population.
- Identify organizations in our communities to partner with on implementing a social determinants of health screening tool.
- Organize and help facilitate meetings between SHR staff and community partners: 2 group presentations and 4 focus groups to identify, develop, and implement a SDOH screening tool used throughout the network of partnering organizations.
- Work with other SHR departments to integrate CAB input into the development of the SDOH screening tool.
- Coordinate the development of a referral network that can be integrated into existing EMR systems so that community organizations may respond to the needs of clients identified by the SDOH screening tool.
- In coordination with key individuals develop assessment tools to track, analyze and present patient outcome measures.
- Evaluate of the effectiveness of the screening tool, and the referral system, to ensure that clients are being connected to appropriate services.
QUALIFICATIONS:
- Requires valid Texas Driver’s License, appropriate driving record and auto liability insurance
- Current BLS certificate
- Experience with EHR (electronic health records) preferred
