What You'll Do
- Conduct patient assessments and develop individualized care plans
- Coordinate services with physicians, therapists, caregivers, and community resources
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on medications, treatment plans, and wellness strategies
- Maintain accurate clinical documentation and regulatory compliance
Qualifications
- Current Minnesota Registered Nurse (RN) license required
- Home health, case management, transitional care, or related clinical experience preferred
- Strong assessment, communication, and care coordination skills
