The Patient Flow Specialist plays a critical role in supporting multiple Transitional Care Units (TCUs) across Ontario during evening hours. This part-time position (20 hours per week) is ideal for a regulated health professional who brings strong clinical expertise, sound judgment, and a systems-focused approach to improving patient transitions across care settings.
Reporting to the immediate supervisor, the Patient Flow Specialist is responsible for supporting the delivery of high-quality, patient-centered care while ensuring compliance with internal policies and external standards of practice. This role has a strong operational focus and supports timely decision-making related to patient flow, admissions, and discharges during evening periods.
This position requires a proactive, collaborative, and flexible approach. The successful candidate will work closely with interdisciplinary teams and hospital partners to assess patients, coordinate admissions and discharges, and proactively address barriers to flow across multiple TCUs. The Patient Flow Specialist helps ensure patients are in the most appropriate setting at the right time, even during non-standard hours.

What You’ll Be Doing
- Manage and support admissions, discharges, and transfers across multiple TCUs during evening hours to ensure seamless transitions and optimal bed utilization
- Act as a key point of contact for hospital partners and internal care teams to support timely, patient-centered decision-making
- Apply critical thinking and creative problem-solving to identify and remove barriers to flow, manage risks, and escalate issues appropriately
- Provide guidance and support to on-site teams through coaching, virtual or in-person rounding, and collaborative problem-solving
- Support consistency in processes and contribute to continuous improvement initiatives that enhance system responsiveness and flexibility
- Degree or diploma in Nursing, Social Work, Social Service Work, or an equivalent combination of education and experience
- Five to ten years of clinical experience in healthcare settings, ideally with exposure to patient flow, transitions, or acute/complex care environments
- Strong stakeholder management, facilitation, and decision-making skills, with the ability to work independently during evening hours
- Comfort supporting multiple sites and navigating system-level flow challenges
- Part-time evening role (20 hours per week) offering flexibility
