Location: Sussex Campus Hospital

Status: Full Time 80 Hours Biweekly

SALARY RANGE: 16.98 - 25.48HOURLY

General Summary:

The Certified Peer Recovery Specialist is a skilled and compassionate professional with lived experience. The Certified Peer Recovery Specialist will report to Care Management leadership and work within an interdisciplinary team, including Care Management, Behavioral Medicine, and our Emergency Department (ED) team members. This team member will utilize their own recovery experience and peer recovery training to enhance patient motivation and assist them in developing the skills and knowledge necessary for their recovery. The team member will also play a key role in improving the overall care of Bayhealth's substance abuse populations, supporting their journey toward recovery, and facilitating their successful transition back to the community. The team member will utilize Motivational Interviewing skills with a variety of patients and family members who present to the ED and on our inpatient medical floors with a substance abuse diagnosis or substance abuse-related illness. Part of the team member’s regular duties will also be to follow up with patients who are admitted to acute medicine (inpatient medical) floors from our ED or coordinate care with Behavioral Medicine team members.

Team member must be willing to self-disclose lived recovery experience for the purpose of modeling hope, personal responsibility, and self-advocacy to demonstrate how recovery goals can be respectfully met and how a belief in self is maintained.


Responsibilities:

1. As a part of the Community Transition and Recovery Team, work with Care Management, Behavioral Medicine, and our ED teams to provide support to Bayhealth’s substance-abusing populations.
2. Provides peer support to engage individuals in a recovery partnership and assists in identifying participants’ recovery goals and the solutions they are seeking.
3. Assist individuals with the development of a personalized crisis plan, recovery action plan, and person-centered treatment plan.
4. Maintain a caseload and prepare narrative reports as required.
5. Promote self-advocacy by assisting recovering persons to have their voices fully heard and have their needs, goals, and objectives established as the focal point of rehabilitation and clinical services.
6. Assist in developing and cultivating connectedness in the client’s life (to self, family, friends, clinical home, treatment services provider, community, and meaning and purpose).
7. As a part of the Community Transition and Recovery Team, advocates for the clients to identify services or resources to help the individuals achieve their goals.
8. Conducts routine follow-up engagement to maintain contact with clients.
9. Assist Behavioral Medicine, Care Management, and ED Teams in SDOH and substance abuse screening and risk assessment in the ED and on inpatient medical floors.
10. All other duties as assigned within the scope and range of job responsibilities.

Required Education, Credential(s) and Experience:

  • Education: High School Diploma or GED
    ;

  • Credential(s): Certified Peer Recovery Specialist
    ;
  • Experience: Required: Twelve (12) months of working or volunteering, under supervision, in a peer recovery specialist role with a minimum of 3 months of narrative report writing experience.
