BayCare Health Systems is currently seeking a Transition Care Specialist to join our outstanding and compassionate team at North Bay Recovery Center !

- Shift Hours: 8:00 am- 4:30 pm
- Weekend Work: Occasional
- On Call: No
The Transition Care Specialist responsibilities include:
- Responsible for ensuring timely, accurate, and HIPAA-compliant transmission of critical patient discharge documentation to the next level of care.
- This role supports continuity of care by coordinating and documenting the transition process for discharged patients from BayCare facilities, with a focus on meeting HBIPS (Hospital-Based Inpatient Psychiatric Services) quality measures.
- The Specialist reviews discharged patient lists, verifies next-level providers, compiles required documents, submits medical record requests, and monitors follow-up to guarantee seamless patient transitions. This position is essential to maintaining compliance with regulatory agency requirements, including The Joint Commission and CMS, which mandate timely and accurate transitions of care for behavioral health patients.
Required Certification:
BLS (Basic Life Support)Required Experience:
- 1–2 years in a healthcare setting in Medical records OR health information management OR discharge planning OR Care coordination OR case management behavioral health OR psychiatric facility administration
Required Education:
- Bachelor’s degree in Health Information Management (HIM) OR Healthcare Administration OR Behavioral Health/Psychology/Social Work/Public Health.
