The Social Services Specialist responsibilities include:
- Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
- The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
- They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
- Possess a clear understanding of discharge and transition planning and linkage with community resources Demonstrates skills in planning, organizing, and managing multiple functions and complex processes
Position details:
- Location: Mease Countryside Hospital - Safety Harbor, FL
- Status: Part time, 16 hours per week
- Schedule: Saturday & Sunday 8:00 AM - 4:30 PM
- Weekend requirement: Every On Call: No
Certifications and Licensures:
- Preferred ACM (Case Management) or Preferred CCM (Case Manager)
Education:
- Required Masters in Social Work, Sociology, or Related Field Strongly Preferred Masters Degree in Social Work
Experience:
- Required 6 months Internship or Required 1 year Social Work in an acute care or hospital setting
