Home Health Aide Per Visit - Private Duty

BayCare is currently in search of our newest Home Health Aide Per Visit who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

Position Details:

• Facility: BayCare Health System, Private Duty
• Location: Pinellas County
• Status: Per Visit, hourly
• Shift Hours: 7:00 AM - 7:00 PM, Varies
• Weekend Work: Every 3rd weekend
On Call: No

Home Health Aide key responsibilities:

• As part of the patient care team, works under the guidance and supervision of the staff nurse in providing personal care to the patients
• Follows the patient's plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional
• Maintains an open communication process with the patient, representative (if any), caregivers, and family
• Demonstrates competency with assigned tasks
• Complies with infection prevention and control policies and procedures
• Reports changes in the patient's condition
Honors patient rights

Required Certifications and Licensures:

• CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• BLS (Basic Life Support)

Education:

• Preferred: High School Diploma or equivalent

Experience:

• Preferred 1 year Home Care
