What are the responsibilities and job description for the Faith Community Nurse Coordinator position at BayCare?

Responsibilities:
- Supports the development, implementation, and evaluation of faith community nursing programs in assigned communities.
- Serving as a liaison between the healthcare system and local faith communities, the coordinator guides both paid and volunteer Faith Community Nurses (FCNs) in providing holistic care that integrates physical, emotional, and spiritual well-being.
- Key responsibilities include recruiting and orienting FCNs, assessing health needs, coordinating and performing wellness screenings and education events, with presentations to nurses and clergy.
- Maintains program data, ensures compliance, and fosters partnerships with congregations and community organizations to align services with BayCare’s health priorities and Community Health Needs Assessments (CHNAs).
- Responsible to coordinate care of the clients enrolled in the FCN Transitional Care Program. Other duties as assigned.
Minimum Qualifications:
Required Education: Associate's degree in Nursing or Diploma Nursing. Preferred Education: BSN (Bachelor of Science in Nursing).
Required Certification: BLS (Basic Life Support) and RN (Registered Nurse).
Required Experience: Minimum 3 years of direct patient care experience. Working knowledge and computer proficiency (Microsoft suite).
