- Location: Bradenton, FL
- Status: Full Time
- Shift Hours: 8:00 AM - 5:00 PM / Monday - Friday
- Weekend Work: 1 weekend rotation a month
- On Call: Yes
- Home visits in assigned service area: Manatee County, FL

The Home Health LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:
- Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse
- Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse
- Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status
- Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.
Required Certifications and Licensures:
- BLS (Basic Life Support)
- LPN (Registered Nurse) License
- Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
Education:
- Required Associate's or Diploma Nursing
Experience:
- Required 1 year Licensed Practical Nurse
- Preferred 1 year Home Care
