This position involves performing outpatient home health visits for adult and geriatric patients in homes throughout Burlington County and Camden County, NJ.
Responsibilities
- Evaluate patients to develop a plan of care for maximizing functional independence.
- Perform evaluations, treatments, and proper discharge planning using an outpatient EMR platform.
- Work closely with healthcare partners to transition care from home health to outpatient services.
- Educate clients, families, and caregivers on care plans.
- Travel to cases as assigned.
Requirements
- A current license as a Speech Language Pathologist in NJ.
- A minimum of one year of recent work experience as a Speech Therapist.
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association.
- Previous home healthcare experience welcomed, but not required.
- Ability to deliver patient care with compassion, excellence, and reliability.
