Job Summary
We are seeking a dedicated and compassionate Chemical Dependency Counselor to join our team. This role requires a commitment to providing high-quality care, conducting assessments, and developing treatment plans tailored to each client's needs.
Responsibilities
- Conduct comprehensive assessments and evaluations of clients to determine their needs and develop individualized treatment plans.
- Provide counseling and support to clients in individual and group settings, facilitating discussions that promote recovery and personal growth.
- Collaborate with multidisciplinary teams to coordinate care and ensure comprehensive support for clients.
- Maintain accurate and up-to-date documentation of client progress, treatment plans, and any changes in condition in compliance with HIPAA regulations.
- Educate clients and their families about addiction, recovery processes, and available resources.
- Monitor clients' progress throughout the treatment process, adjusting plans as necessary based on their evolving needs.
- Participate in case management activities, including discharge planning and referrals to community resources.
- Stay informed about current trends in addiction treatment, including evidence-based practices and emerging therapies.
Qualifications
- Bachelor's degree in psychology, social work, counseling, or a related field; Master's degree preferred.
- Relevant experience in behavioral health or substance abuse counseling is highly desirable.
- Familiarity with EMR/EHR systems such as Epic or Cerner is advantageous.
- Knowledge of ICD-9/ICD-10 coding for accurate medical documentation.
- Experience working with individuals with developmental disabilities or those requiring memory care is a plus.
- Strong understanding of case management principles and practices within chemical dependency treatment settings.
- Ability to perform patient assessments, including vital signs monitoring and medical history intake.
- Excellent communication skills, both verbal and written, with the ability to engage effectively with clients from diverse backgrounds.
- Commitment to ongoing professional development in areas such as pain management, trauma medicine, or hospice & palliative medicine is encouraged.
