Job Summary
We are seeking compassionate and dedicated Direct Care Workers to provide high-quality support and assistance to individuals with various needs, including seniors, individuals with disabilities, and those requiring specialized care such as dementia or Alzheimer's. This role involves working closely with clients to promote independence, safety, and well-being within their homes or community settings. The ideal candidate will have a strong commitment to patient-centered care and possess relevant experience in home health, assisted living, or long-term care environments.
Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting
- Provide medication administration and monitor vital signs as required
- Support patients with mobility using equipment such as Hoyer lifts and assist with transfers
- Prepare nutritious meals in accordance with care plans and dietary restrictions
- Observe and document patient conditions, including changes in behavior or health status
- Maintain a clean and safe environment through regular cleaning and sanitation practices
- Implement individualized care plans tailored to each resident’s needs
- Engage clients in social activities to promote mental and emotional well-being
- Ensure compliance with HIPAA regulations to protect patient confidentiality
- Respond promptly to emergencies with appropriate first aid or CPR when necessary
- Collaborate with healthcare professionals, family members, and support teams to deliver comprehensive care
Skills
- Experience working in group homes, assisted living, nursing homes, or home & community care settings
- Knowledge of Alzheimer’s, dementia, memory care, autism spectrum disorders, and hospice care is highly desirable
- Ability to work effectively with individuals with developmental disabilities or other special needs
- Strong patient observation skills for monitoring health status and behavioral changes
- Proficiency in assisting with ADLs, medication management, and vital signs monitoring
- Ability to perform heavy lifting safely when necessary for resident transfers or mobility assistance
- Familiarity with HIPAA regulations and maintaining confidentiality of patient information
- Excellent communication skills for engaging with clients, families, and healthcare teams
- Compassionate attitude coupled with patience and empathy for diverse populations
- Basic cooking skills for meal preparation aligned with dietary guidelines
