Essential functions of position. (Reason position exists is to perform these functions.) List duties that must be performed to accomplish the purpose of the position.
- Accurately record and measure vital signs (blood pressure, pulse, blood glucose, height, weight) for clinic patients.
- Assist the clinic medical provider with direct client care as needed or instructed.
- Assist the clinic medical provider with coordination with PCP offices and other medical providers as directed.
- Telephone screenings—Triage phone calls from clients, obtaining information regarding client concerns, communicating with RN/ LMP and accurately relaying directions back to clients.
- Refilling of medications as directed by LMP.
- Effectively and accurately document client contacts and interventions
- Conduct AIMS exam, documents findings and report abnormalities to LMP
- Prepares clients for clinic services; performs initial health screening and lab
- Coordinate care with agencies such as Translink, Dial-a-ride, AFH.
- Work in conjunction with case managers and therapists in coordinating client care.
- Perform Point of care lab testing as directed
- Administration of injections as ordered by medical provider ie., Vaccines
- Assist in tracking internal referrals and scheduling of patients for clinic.
- Effectively and accurately document client contacts/interventions.
- Complete note and care plan in patient’s electronic health record.
- Complete prior authorization requests
- Utilize Collective Medical to identify patients with multiple ER visits.
- Update Collective Medical care plan
- Ability to adhere to strict policies regarding client confidentiality (HIPAA).
- Other duties appropriate to classification, as assigned.

- Attend staff meetings at direction of supervisor.
- Keep medical supplies stocked and Exam rooms clean
- Assist physically disabled clients to and from clinic appointments (within the building).
- Provide support services to Clinic team, i.e. copying, printing, stocking, and typing.
- Other duties appropriate to classification, as assigned.
- Embraces the performance management system, QI and Workforce development efforts
List required special skills, licenses, certificates, etc.
- High school diploma required. College course work preferred, particularly courses related to psychology/mental illness. Experience of at least one year in the mental health field preferred.
- Demonstrated skill in dealing with the mentally ill and their families is preferred.
- American Red Cross CPR/Basic First Aid is required within 60 days of hire.
- Preference may be given to candidates with advanced knowledge of medical terminology.
- Basic office skills, including: typing, computer skills, knowledge of Electronic Health Record charting. Must be capable of working in a Windows software environment. Must be able to learn/utilize computer medical record system, including use of newer technologies.
- Knowledge of scheduling functions of computer software for Clinic appointments.
- Basic knowledge of mental illnesses: course work or trainings related to mental illness preferred.
- Understanding of DSM/ICD-10/CPT coding is helpful.
- Must have good time management skills; ability to prioritize tasks and work in a fast-paced environment; good writing/composition skills. Must have ability to prepare concise and complete reports and maintain client records.
- Good organizational skills required, including the ability to develop and maintain tracking systems for medical team functions.
- Must have good communication skills; must be able to function within a team environment; must be able to effectively communicate with other medical team members and supervisor.
- Must have ability to prioritize tasks, giving priority to more urgent clinical concerns.
- Must be able to effectively communicate with clients, maintain personal boundaries with clients and accurately report information between clients and other medical staff.
- Regular and consistent attendance is required.
