What are the responsibilities and job description for the Community Outreach Worker - B'more for Healthy Babies position at BALTIMORE MEDICAL SYSTEM, INC?
– Functions as a patient advocate through diverse community outreach efforts in East Baltimore
– Identifies potential outreach targets and develops creative and effective ways to reach these targets
– Generates new clients for BMS through contacts with individuals and families currently not connected to primary care
– Educates community members about resources and sources of care, particularly with regard to those intended for mothers and babies
– Works to remove barriers to care by direct contact and relationship building with identified clients and referral agencies
– Collects relevant data and generates reports on outreach activities for use internally and to meet grant maker requirements
– Represents BMS and B’more for Healthy Babies at a minimum of 8 community activities monthly
– Represents BMS and B’more for Healthy Babies on a minimum of one Community Board meeting at least quarterly
– Offers presentations both group and individual on B’more for Healthy Babies educational themes throughout the BHB catchment area in Patterson Park North and East
– Participates monthly in the B’more for Healthy Babies Collaborative for Patterson Park North and East
General duties:
– Consistently and independently follows established safety and infection control procedures. Intervenes in unsafe situations.
– Complies with the standards of conduct and Corporate Responsibility Program and seeks guidance when in doubt.
– Meets or exceeds expectations of individuals for whom service is provided.
– Participates on interdepartmental or departmental committees as appropriate.
