The Community Health Worker – HIV serves as the liaison for BMS patients living with HIV, HCV or taking PrEP, BMS as an organization, and community services. This center-based position will provide accurate information to clients about living with HIV, preventing further transmission, the benefits and challenges of HIV care, Pre-exposure Prophylaxis (PrEP) and available services. This position participates in community events to increase community awareness of BMS programs, expand HIV and HCV testing and treatment, educate the public on HIV/HCV/PrEP and work to reduce related stigma. They will provide evidence-based health guidance and social assistance to BMS patients and other community members. This CHW-HIV will provide informal counseling, social support, and appropriate referrals, in consultation with clinical staff when appropriate, to ensure patients have access to services needed. They will provide care support, follow-up, and education in clinical and community settings. By identifying and addressing issues that create barriers to care for patients, they will facilitate patients’ ability to receive services. Once a patient has related to services, they will follow up with the patient in person and/or via telephone to ensure the patient was able to fully access services and have their issues addressed. During their job, they will collect relevant data on their activities and patients and report that data in the means required by BMS. Finally, they will participate in all team meetings relevant to the assigned program.
