- Serving as the primary clinical partner for patients and families
- Performing comprehensive in-home assessments and developing individualized care plans
- Administering skilled interventions such as wound care, infusions, medication management, and post-operative care
- Educating and empowering patients and caregivers for symptom and medication management
- Monitoring patient progress, identifying early changes, and communicating with care teams
- Coordinating care across providers to ensure continuity and quality outcomes
- Maintaining accurate, timely documentation to support compliance and quality measurement
- Active RN license in the practice state
- At least one year of clinical nursing experience
- Patient-focused, organized, and motivated to deliver high-quality care
- Valid driver’s license, reliable transportation, and CPR certification or ability to obtain
