Role: RN Care Coordinator responsible for assessing patient needs, coordinating care across the continuum, and facilitating discharge planning to ensure efficient transitions and meet patient care goals.
Responsibilities: Conduct psychosocial and clinical assessments, develop individualized discharge plans, collaborate with physicians and the care team, manage barriers to discharge, and facilitate post-acute referrals. Participate in interdisciplinary rounds, document care activities accurately, and support patient and family education. Proactively identify delays in care or diagnostic procedures, resolve obstacles to discharge, and ensure compliance with policies and regulations.
Qualifications & Skills: Valid RN license (single or multi-state), BLS certification, minimum of 1 year acute care nursing experience, excellent communication, assessment, organizational, and problem-solving skills. Ability to work collaboratively, handle stress, and adapt in a fast-paced environment. Knowledge of regulations and community resources is essential.
