Responsibilities
- Perform comprehensive assessments, including medical history, vital signs, physical exams, screenings, and point-of-care testing
- Identify diagnoses, document findings, and communicate with primary care providers
- Recognize urgent situations and intervene appropriately
- Educate members on health management, medications, and social determinants of health
- Provide referrals and address social needs as necessary
- Active licensure as a Nurse Practitioner (ANCC/AANP) or Physician Assistant (NCCPA, state license)
- Unrestricted driver's license and reliable transportation
- Ability to navigate home environments and perform physical assessments
- Preferred: 1 years clinical experience, proficiency with electronic records, strong communication skills, and ability to handle equipment during home visits
