- Deliver patient-centered care based on individualized plans, addressing physical, emotional, cultural, spiritual, and psychosocial needs.
- Monitor and document patient status, including vital signs, drains, dressings, and lab values.
- Communicate patient observations and changes to healthcare team members.
- Maintain accurate, confidential medical records.
- Use specialized equipment for data collection and orient patients to routines.
- Administer and document medications according to policy.
- Perform additional duties as assigned.
- Completion of an accredited LPN program recognized by the State Board of Nursing.
- At least one year of LPN experience or equivalent training, such as residency programs or LPN-to-RN transition programs.
- Current LPN licensure recognized by the NCSBN Compact or applicable state licensing.
- Ability to perform delegated tasks and communicate effectively.
