- Educating patients and caregivers to manage symptoms and treatments confidently.
- Monitoring patient progress, identifying changes, and collaborating with healthcare teams to adjust care plans.
- Ensuring care continuity and completing accurate documentation to support quality outcomes.
- Active RN license in the practice state.
- At least one year of clinical nursing experience.
- Patient-focused, organized, and motivated to deliver high-quality care.
- Valid driver’s license, reliable transportation, and CPR certification (or ability to obtain).
