Job Title: Community Resource Navigator
Pay: $24-$27/hr
Location: Onsite, Santa Rosa, CA
Type: Contract – 13 weeks (possible temp-to-perm)
Schedule: Mon-Fri 8a-4:30p
Minimum Qualifications
- High School Diploma or equivalent
- Minimum of 1–2 years of experience in healthcare, case management support, discharge planning, insurance verification, utilization review, or related healthcare administrative role preferred.
- Experience working with community resources, referrals, insurance authorizations, or care coordination strongly preferred.
- Knowledge of Medi-Cal/Medicaid, Medicare, managed care, and payer authorization processes preferred.
- Knowledge of medical terminology, healthcare systems, discharge planning, and community resources.
Position Summary
The Community Resource Navigator supports the case management and continuum of care process by coordinating administrative, referral, discharge planning, and community resource activities to ensure timely, efficient, and appropriate patient care transitions. This role serves as a liaison between patients, families, providers, payers, and community agencies to facilitate continuity of care, authorizations, referrals, and access to needed services and benefits.
Key Responsibilities
- Supports the case management and continuum of care process through coordination of referrals, transfers, discharge planning, and community resources.
- Coordinates services including home health, durable medical equipment (DME), oxygen, transportation, and referrals to outside facilities or lower levels of care.
- Obtains insurance verifications, authorizations, benefit information, and assists with Medi-Cal applications, non-coverage letters, appeals, and denial tracking.
- Serves as a resource for patients, families, physicians, hospital staff, and community providers regarding discharge planning and available resources.
- Facilitates communication and exchange of patient information to support continuity of care and timely transitions.
- Maintains accurate documentation, data entry, medical records, utilization review requests, and claims-related information.
- Assists with department administrative functions including scheduling support, special projects, and participation in meetings and committees.
- Ensures compliance with organizational policies, regulatory requirements, and standards of confidentiality and patient care.
