Licensed Social Worker (Skilled Nursing Facility)

Job Summary
We are seeking a compassionate and dedicated Licensed Social Worker who is passionate about improving the lives of residents. In this role, you will assess and address the psychosocial, emotional, and mental health needs of our residents while ensuring access to appropriate services.
All services will be delivered in accordance with the National Association of Social Workers (NASW) Code of Ethicsand in compliance with all federal, state, and local regulations.
Qualifications
Licensed, certified, or registered Social Worker in the state of practice (required)
Bachelor’s Degree in Social Work or related Human Services field (e.g., Sociology, Psychology, Special Education, Rehabilitation Counseling) from an accredited institution (required)
Minimum 1 year of supervised social work experience in a healthcare setting with geriatric populations (required)
2 years of nursing experience in a Skilled Nursing Facility (preferred)
Key Responsibilities
Complete comprehensive social histories and psychosocial assessments for each resident
Contribute to and utilize the Minimum Data Set (MDS) to guide individualized care planning
Maintain accurate, timely, and compliant documentation, including:
Care plans, assessments, progress notes
Advance directives, PASARR, discharge planning
Behavioral tracking and psychosocial monitoring
Advocate for and uphold resident rights, including self-determination and informed decision-making
Participate in developing and implementing interdisciplinary care plans
Provide therapeutic support to help residents adjust to long-term care environments
Educate and support residents, families, and caregivers regarding care and available resources
Coordinate discharge planning and community referrals
Collaborate with the interdisciplinary team to promote resident well-being and safety, including abuse prevention
Assist with survey readiness and regulatory compliance
Support the Social Services department in program development and improvement
