Rate of Pay: Full time - $38-$53 an hour. Based on years of experience.

PRN - $53-$60 an hour. Based on years of experience.

Job Summary:

Aviata Health Group is seeking a dedicated and compassionate Physical Therapist to join our team at our Tampa, Florida location. As a Physical Therapist at Aviata, you will play a vital role in delivering exceptional, person-centered care to our residents. Responsibilities:

• Develop and implement individualized plans of treatment for each patient based on initial and ongoing evaluations, including short- and long-term goals
• Implement treatment plans on a consistent schedule and adjust plans according to residents' progress
• Document residents' progress according to approved medical documentation and facility documentation procedures
• Evaluate residents' mobility, strength, and current level of function, setting goals to improve these areas
• Report resident progress to residents, caregivers, loved ones, facility staff, and healthcare professionals involved in ongoing treatment/care
• Assist other facility staff members as needed to provide care and create a clean, orderly, and effective environment for rehabilitation and physical therapy
  
  
  

Qualifications:

• Graduate of School of Physical Therapy
• Minimum of three months recent experience as a Physical Therapist working in a nursing home environment preferred
• State of Florida Physical Therapy License
• CPR Certification
