- Evaluate and treat speech, language, cognitive-communication, and swallowing disorders in adult and geriatric populations
- Develop and implement individualized, evidence-based treatment plans
- Collaborate with an interdisciplinary care team including nursing, PT, OT, and social services
- Complete documentation in compliance with facility, state, and federal guidelines
- Educate residents, families, and staff on communication strategies and safe swallowing techniques
- Participate in care plan meetings and discharge planning as appropriate
- Master’s degree in Speech-Language Pathology from an accredited program
- Current or eligible SLP license in the state of Ohio
- CCC-SLP from American Speech-Language-Hearing Association preferred
- CFY candidates strongly encouraged to apply
- SNF or long-term care experience preferred, but not required
- Strong communication, organizational, and clinical reasoning skills
