The Internal Admissions Coordinator plays a key role in managing the admission process for residents entering the nursing home from hospitals, assisted living facilities, or other internal sources. Key Responsibilities:
- Coordinate all aspects of the internal admissions process, including pre-admission assessments, bed availability, and room assignments.
- Serve as the primary point of contact for hospital discharge planners, physicians, residents, families, and internal staff regarding new admissions.
- Review medical documentation and insurance coverage to determine eligibility and appropriate placement.
- Work closely with the clinical and financial teams to ensure the facility can meet the resident’s needs.
- Schedule and conduct admission tours and interviews with prospective residents and families.
- Facilitate completion of all required admission documentation and ensure accuracy and compliance.
- Maintain up-to-date records in the admissions tracking system and produce regular reports for management.
- Assist in promoting a positive image of the facility to the community, potential residents, and referral sources.
- Address questions or concerns from residents and families during the admission process.
- Monitor and support resident satisfaction during the initial transition period.
- High school diploma or equivalent required; Associate or Bachelor's degree in healthcare administration, social work, or related field preferred.
- Minimum of 1–2 years experience in healthcare admissions, case management, or related field; nursing home experience highly preferred.
- Knowledge of Medicare, Medicaid, and private insurance guidelines.
- Strong communication, organizational, and interpersonal skills.
- Ability to work in a fast-paced environment and manage multiple priorities.
- Compassionate and professional demeanor with a resident-centered approach.
