Position Summary
The Wellness Director is responsible for planning, organizing, directing, and evaluating the overall health and wellness programs for residents in a long-term care facility. This role ensures the delivery of high-quality resident-centered care in compliance with federal, state, and local regulations, while promoting residents’ physical, emotional, and social well-being.
Key Responsibilities
1. Clinical Leadership & Oversight
Oversee daily clinical operations, including nursing and wellness services.
Ensure individualized care plans are developed, implemented, and updated.
Monitor residents’ health status and coordinate with physicians, therapists, and other healthcare providers.
Conduct regular health assessments and wellness checks.
Ensure accurate and timely clinical documentation.
2. Regulatory Compliance
Maintain compliance with all federal, state, and local regulations (e.g., Centers for Medicare & Medicaid Services (CMS), state Department of Health).
Prepare for and participate in surveys and inspections.
Develop and maintain policies and procedures consistent with regulatory requirements.
Oversee infection control and safety protocols.
3. Staff Management
Recruit, hire, train, supervise, and evaluate nursing and wellness staff.
Ensure appropriate staffing levels to meet resident needs.
Provide ongoing education and professional development opportunities.
Foster a positive, collaborative work environment.
4. Resident & Family Engagement
Communicate regularly with residents and families regarding care plans and health concerns.
Address and resolve resident or family concerns promptly.
Promote wellness initiatives that enhance quality of life.
5. Quality Assurance & Performance Improvement
Lead Quality Assurance and Performance Improvement (QAPI) initiatives.
Monitor clinical outcomes and implement corrective action plans as needed.
Track key performance indicators such as falls, medication errors, infections, and hospital readmissions.
6. Budget & Resource Management
Assist in developing and managing the department budget.
Monitor medical supplies and equipment inventory.
Ensure cost-effective use of resources without compromising care quality.
Qualifications
Education
Graduate of an accredited school of nursing (BSN preferred).
Current Registered Nurse (RN) license in good standing (state-specific).
Experience
Minimum 3–5 years of clinical leadership experience in long-term care, skilled nursing, or assisted living.
Experience with electronic health records (EHR).
Strong knowledge of geriatric care practices and regulatory standards.
