WORK SCHEDULE
Variable days and hours, including holidays and occasional weekends.


POSITION SUMMARY
Under the direction and supervision of the Director of Social Services, the Discharge Planner works closely with the Clinical Team to provide critical support and care coordination for our patients to prepare them for discharge from our hospital. The Discharge Planner works diligently to ensure that patients have clinically appropriate and safe discharge planning, as well as providing recommendations for post-discharge support.

QUALIFICATIONS


EDUCATION
Minimum education requirements include an Associate Degree in a related field, or HS Diploma and at least 3 years of relevant experience.


CERTIFICATION/LICENSES
CPR and CPI Certified per policy


EXPERIENCE
Three years clinical experience in a psychiatric setting, with at least one year’s
experience being involved with, or assisting with patient discharge planning.

Ability to: provide services as described above; perform physical interventions if/when needed; document clearly in the electronic medical record; use the fax, email, telephone, computer keyboard and security door keypad or keys.

Ability to effectively read, write and process information accurately; input/retrieve information to/from the computer, fax, phone and patient records; see objects and persons near and at a distance.


HEARING
Ability to: communicate effectively with others and to give, take and process information; use the telephone extensively; ability to hear patients and staff on the unit so response can be timely and appropriate to needs; hear alarms or
other sounds that may indicate a safety concern.


WORKING CONDITIONS
Exposure to: infection from disease-bearing specimens; blood-borne pathogens, body fluids and wastes; hazardous anesthetic agents; occasional exposure to unpleasant patient or departmental elements; occasionally subjected to irregular or prolonged hours; occasional exposure to high risk behaviors from patients or
visitors that may result in agitation, aggression or violence; exposure to critical incidents and possible exposure to sentinel events.


ESSENTIAL JOB FUNCTIONS


PROGRAM SERVICES
  • Provide therapeutic activities for assigned patients as scheduled.
  • Assess patient needs and facilitate activities that are appropriate to age, development, needs and interests of the patients served.
  • Document group sessions in the medical record per hospital requirements.
  • Seek clinical supervision and support when complications arise in dealing with a patient(s) either in an individual or group setting.


TREATMENT TEAM MEMBER
  • Provide services as a member of the multidisciplinary team and as directed by the attending physician, the Director of Social Services and the patient treatment plan.
  • Communicate with nursing staff any changes in a patient’s status, including medical or behavioral.


CUSTOMER INTERACTION
  • Interact with patients, family members, external entities and internal staff professionally and respectfully.
  • Work collaboratively with all treatment team members to create a treatment environment that supports the hospital’s mission to provide exemplary patient care and customer service.
  • Act to preserve patient and family dignity and safety.


DOCUMENTATION
  • Documentation for medical records and reports is timely, legible, thorough, and in required format.
  • Document as required including individual case management notes, treatment plan, and group notes.
  • Update and revise treatment plan as assigned and per policy.
  • Results of the assessment shall be signed by the therapeutic recreation evidencing approval of such results.


DISCHARGE PLANNING
  • Assume an active role in discharge planning by identifying barriers to discharge during initial assessment and ongoing interactions with the patient and family.
  • Formulate discharge plan in coordination with the treatment team, patient, family, and outside agencies as appropriate.
Establish and maintain cooperative relationships with community agencies and other resources.
