What are the responsibilities and job description for the Hospice CNA position at Autumn Hospice?
Position Summary: The Certified Nursing Assistant will carry out assigned duties which promote comfort and support to Individuals and their families.

Essential Job Functions and Duties:
· Provide personal care and hygiene to Individuals under Autumn Hospice.
· Assist Individuals with activities of daily living.
· Change bed linens and straighten Individual’s environment.
· For Individuals in a home setting, clean bathroom and kitchen and do personal laundry, as requested.
· Maintain a minimum Productivity Standard according to Autumn Hospice Policy. Productive time consists of face-to-face contact with Individuals and families in the form of direct care activities.
· Observe and report all changes in Individuals’ conditions and needs to the RN Case Manager and/or facility Unit Manager.
· Follow Individual Plan of Care at all times. Ensure that all Plan of Care documentation is entered into EMR accurately, timely, and appropriately.
· Communicate all aspects of Individual care with SNF/caregivers.
· Attend Interdisciplinary Team (IDT) meetings when requested. Communicate appropriate Individual information to the IDT.
· Participate in Autumn Hospice Quality Assurance/Performance Improvement program.
· Wear identification badge and smock and maintain professional appearance at all times.
· Follow HHA schedule as written by RN Case Manager and/or LPN and report schedule change needs to RN Case Manager and/or LPN promptly.
· Attend and participate in all in-service trainings and meetings as needed.
· Ensure that all care is provided with respect for Individual rights.
· Report all accidents and incidents observed. Identify and respond appropriately to emergency situations.
· Maintain all Individual privacy and confidentiality in accordance with Autumn Hospice policies and HIPAA requirements.
· Ensure that all care is provided consistently with respect for Individual rights. Report any/all allegations of Individual abuse, neglect, and/or misappropriation of property in accordance with North River policies.
· Maintain required clinical competency/license as appropriate.
· Complete other duties as assigned.
Position Qualifications and Entrance Requirements:
· Certification as a Certified Nursing Assistant is required
· Minimum of 1 year experience as a Home Health Aide or C.N.A. is required
· Solid verbal, written, and interpersonal communication skills are required.
· Experience working independently with minimal supervision is required.
· Candidates must successfully complete criminal background check and physical (including drug test & TB screening) prior to completing Orientation.
Preferred skills and/or experience:
· High school diploma or GED preferred
· Previous experience in hospice preferred
Physical Demands/Work Environment:
· Must be able to lift up to 50 pounds. Frequent lifting and/or carrying of objects weighing up to 25 pounds.
· Must be able to travel to facility sites on a daily basis
· Must be able to complete physical activities (walking, standing, talking, hearing, pushing, reaching, seeing, stooping/crouching, smelling, depth perception, identifying colors) as needed.
· Must be able to travel to Individual locations on a daily basis
· Must be able to work with exposure to possibly disagreeable conditions including but not limited to: odors, infection, illness, and physical contact with disruptive Individuals.

Pay: $22.00 - $26.00 per hour
