MDS Nurse
Pay: $33.00–$40.00 per hour
Austinwoods Nursing Center is seeking an experienced Registered Nurse (RN) or LPN for our MDS program.

Job Functions
- Coordinate and complete MDS assessments in accordance with CMS and regulatory requirements.
- Lead the interdisciplinary team through the MDS and care-planning process.
- Ensure accurate and timely clinical documentation.
- Oversee and coordinate the Restorative Nursing Program.
- Educate and support nursing staff regarding restorative programs.
- Assist with survey readiness and quality improvement initiatives.
Qualifications
- Current Ohio RN license, Preferred.
- MDS experience preferred.
- Long-term care/SNF experience preferred.
- Strong organizational, documentation, and communication skills.
