- Collaborate with an interdisciplinary team to develop and implement individualized rehabilitation care plans.
- Educate patients and their families about treatment processes and recovery strategies.
- Document care and patient progress accurately in the electronic health record (EHR).
- Current Registered Nurse licensure.
- Experience or knowledge in rehabilitative or acute care settings preferred.
- Strong assessment, critical thinking, and communication skills.
- Ability to work effectively within a team and engage in patient education.
