- We are looking for a CNA that wants to grow in a Social Services role
- Ideal for someone working on a Social Work Degree or with Social Work experience
- 32 Hours / week
- Can pick up CNA shifts for more hours

What You'll Do
- Develop, plan, implement, and evaluate social service programs in accordance with state and federal guidelines
- Identify and address residents' social, emotional, and psychological needs
- Provide service coordination for residents and families
- Support care planning and interdisciplinary collaboration
- Ensure compliance with applicable regulations and documentation standards
What You Bring
- Licensed Social Worker preferred, but not required
- At least one year of supervised social work experience in a healthcare setting (elderly psychosocial experience preferred)
- Working knowledge of nursing home regulations, including MDS procedures
- Familiarity with Medicare and Medicaid reimbursement and eligibility guidelines
- Strong written and verbal communication skills
- Positive, can-do attitude with excellent attendance
