What You’ll Do
Assist the Social Services Director with coordinating and implementing social service programs
Support residents with social, emotional, and psychosocial needs
Assist with admissions, discharge planning, and transitions of care
Communicate with residents and families to provide support, education, and resources
Help maintain accurate documentation in compliance with regulatory requirements
Participate in care plan meetings and interdisciplinary team discussions
Advocate for resident rights and ensure concerns are addressed promptly
Assist with referrals to community resources and outside services
Support compliance with federal and state regulations, including documentation standards
What You Bring
Previous experience in healthcare, social services, or long-term care preferred
Bachelor’s degree in social work or related field preferred (not required)
Strong communication and interpersonal skills
Compassionate, service-oriented mindset
Ability to work collaboratively with residents, families, and team members
Organized, detail-oriented, and able to manage multiple priorities
Positive, dependable, and team-focused attitude
Excellent attendance and reliability
